Key Responsibilities Clinical Practice Undertake Structured Medication Reviews (SMRs) for patients with complex needs, polypharmacy, or high-risk prescribing Perform medicines reconciliation post-discharge and following clinic letters to ensure safe continuity of care Provide specialist pharmacological advice to clinicians and support evidence-based prescribing decisions Manage and respond to medication-related queries from patients and external healthcare providers Medicines Management & Prescribing Systems Oversee and continuously improve repeat prescribing processes to ensure safety, efficiency, and compliance Lead the prescriptions team, providing supervision, training, and support Identify and mitigate prescribing risks, implementing appropriate safety measures Long-Term Condition Management Support the clinical management of patients with chronic conditions such as cardiovascular disease, diabetes, asthma, and hypertension Optimise medicines to improve adherence, outcomes, and patient understanding Proactively identify patients requiring medication review or intervention Quality Improvement & Governance Lead and participate in prescribing audits and quality improvement initiatives Contribute to achieving QOF targets and other contractual requirements Ensure compliance with medicines management policies and CQC standards Maintain accurate, high-quality clinical records and documentation Medicines Optimisation Work collaboratively with the Medicines Optimisation Team and Integrated Care Board (ICB) Support initiatives to reduce medicines waste and improve cost-effective prescribing Implement local and national prescribing guidance Professional Responsibilities Maintain registration with the General Pharmaceutical Council (GPhC) Work within professional and ethical frameworks and maintain patient confidentiality at all times Undertake continuing professional development (CPD) and participate in appraisal processes Contribute to the training and development of colleagues where appropriate Key Working Relationships GP Partners and Salaried GPs Practice Manager and administrative team Nursing and healthcare assistant team Community pharmacies Medicines Optimisation Team and ICB colleagues
