Description
We are seeking a compassionate and dedicated ASW Mental Health Social Worker WCR 3 to join our multidisciplinary team and provide critical support to individuals experiencing mental health challenges. In this role, you will be responsible for assessing the social and psychological needs of clients, developing tailored intervention plans, and coordinating care to promote mental wellness and recovery. Responsibilities
Conduct comprehensive psychosocial assessments of clients experiencing mental health issues.
Develop and implement individualized care plans in collaboration with clients and multidisciplinary teams.
Provide crisis intervention and support during acute mental health episodes.
Coordinate with community resources and support services to facilitate client recovery and reintegration.
Maintain accurate and confidential case documentation and records.
Advocate on behalf of clients to ensure access to necessary treatments and social services.
Participate in multidisciplinary team meetings to review client progress and adjust care plans accordingly.
Deliver psychoeducation and support to clients and their families to promote understanding and coping strategies.
Requirements
Bachelor's or Master’s degree in Social Work from an accredited institution.
Current licensure or certification as a Social Worker in the applicable region or state.
Minimum of 3 years experience in mental health social work or a related field.
Strong knowledge of mental health disorders, treatments, and community resources.
Excellent assessment, communication, and interpersonal skills.
Ability to manage crisis situations calmly and effectively.
Demonstrated ability to work collaboratively within multidisciplinary teams.
Proficiency in maintaining detailed and confidential client records using digital systems.
