See JD for full details The Family Worker will work as part of the multidisciplinary professional team within Op Courage - The Veterans Mental Health and Wellbeing Service. This position engages directly with families and carers of UK veterans who are experiencing mental health challenges. The role will deliver safe, effective care with a strong focus on supporting families and carers and fostering therapeutic relationships. The Family Worker will work in partnership with families and carers to ensure that the service users health needs are met. They will demonstrate an understanding of nursing interventions relevant to the care group including health education, clinical care, Wellness Recovery Action Plans (WRAP), and discharge planning To work within the MDT in identifying Op Courage Veterans families with additional needs including mental health, financial and support needs. To be an active participant in the multi-disciplinary team, formulating care plans and utilising appropriate assessments. To take part in all quality initiatives actively supporting and participating in both policies and service developments in the specific care group. Carry a caseload, actively working with family and carers. To co-develop and deliver educational workshops to help develop family members' understanding of common mental health difficulties which their veteran family member may be facing. To co-develop care group specific brief interventions to support family members in their support of Veterans, utilising evidence-based models of practice. The family Worker will be expected to travel around the region as required and deliver both online and face to face work in the community and home visits as appropriate.
