Key Responsibilities Manage a mix of telephone, face-to-face, and home visit consultations Deliver planned and urgent primary care tailored to patient needs Support patients with complex and long-term conditions Provide clinical advice and guidance to the multidisciplinary team Maintain high standards of evidence-based, patient-focused care What We Offer You will be part of a supportive local team, working alongside a skilled multidisciplinary team including Practice Nurses, ACPs, Wellbeing Advisors, Health Coaches, and Physician Associates.
