The Care Coordinator will act as a central point of contact for patients, assisting them in navigating healthcare services and ensuring clear and timely communication between patients, clinicians, and external providers. The post holder will play a key role in supporting the organisation and management of patient care plans, appointments, referrals, and follow-ups, helping to ensure continuity and efficiency in care delivery. The successful candidate will work closely with the wider multidisciplinary team, contributing to a collaborative and supportive working environment. Strong organisational and communication skills are essential, along with the ability to manage competing priorities and maintain a high level of attention to detail. Main Responsibilities Coordinate patient appointments, follow-ups, and care schedules in an efficient and timely manner Act as a point of contact for patients, carers, and families, providing information, support, and guidance Liaise with GPs, nurses, and external healthcare professionals to ensure effective communication and continuity of care Support the management of referrals, ensuring they are processed accurately and within appropriate timeframes Maintain accurate, up-to-date patient records in line with data protection and confidentiality requirements Organise and manage documentation relating to patient care, including treatment plans and correspondence Assist with general administrative tasks such as filing, data entry, and managing incoming and outgoing communications Ensure all work is carried out in accordance with practice policies, procedures, and relevant NHS guidelines Key Skills and Attributes Excellent organisational skills with the ability to prioritise workload effectively Strong communication skills, both written and verbal, with the ability to interact professionally and empathetically Good level of IT competency, including the use of standard office software systems High level of attention to detail and accuracy Ability to work independently as well as part of a team Proactive and flexible approach to work Compassionate and patient-focused attitude
