Community Mental Health Practitioner

Responsibilities include

To contribute to the planning and implementation of interventions. To monitor clients progress, reporting on clients psychological, physical, behavioural and social needs. To adapt and evaluate interventions ensuring that they support the client and his/her carer to meet their needs, adjusting own approach accordingly. To act as lead professional/Care co-ordinator to designated caseload/provide interventions as required
