Applications are invited for a Patient Pathway Coordinator within the Diabetes and Metabolism Department, specifically supporting the Obesity Service at Barts Health NHS Trust, working across The Royal London Hospital, Barts and Mile End Hospital. This role is a one-year fixed term post which will commence from the 1stof April 2026.

This role sits at the heart of the diabetes and obesity care pathways and supports the smooth running of outpatient services in a busy and pressurised environment. You will play a key part in coordinating an efficient administrative function, supporting patients from referral through treatment, discharge, and follow up. Acting as a consistent point of contact for patients, families, clinicians, and wider teams, you will help patients understand their appointments and care journey, responding to queries with clarity and reassurance.

Your day-to-day work will focus on coordinating and validating patient pathways, managing and adjusting clinic appointments, and ensuring all clinic slots are set up and used effectively in line with agreed clinic patterns. You will work closely with clinicians and clinical team leaders to amend, cancel, or reschedule clinics, adjust capacity, and ensure bookings meet local and national targets. Using NHS systems, you will keep records accurate and up to date, monitor waiting lists and cancellations, identify delays early, and escalate risks of breaches.

The postholder is responsible for coordinating patient pathways for diabetes and obesity services, ensuring smooth administration from referral to discharge. The role involves maintaining accurate patient records, managing referrals, communicating with patients and clinical teams, and supporting service performance and compliance with national standards. The postholder acts as the main administrative contact for patients and clinicians across the pathway.

The post holder will be required to work across the Trust’s multiple sites to ensure cross-cover is provided.
