Job Description and Person Specification

• Provide an on-site clinical pharmacy service to the medical day units and outpatient prescribing clinics Support medical and nursing teams by providing pharmacy review and medicines management support to ensure the safe, effective, and timely delivery of oncology services across medical day units, outpatient clinics, ambulatory pathways, and homecare setting. Work flexibly across the day unit, outpatient department, ambulatory unit and clinic areas as required to clinical check the appropriateness of prescribing and promoting medicines optimisation Promote ongoing Trust initiatives to continually improve patient experience. Ensure the appropriate provision of medicationrelated information to patients receiving SACT, including those managed via outpatient, ambulatory, homecare, and selfadministration pathways.
