Job Purpose The Paramedic will play a pivotal role in delivering high-quality, responsive healthcare to patients within the Practice. Working in close collaboration with GPs and the wider multidisciplinary team, the post-holder will provide the first point of clinical triage of all incoming requests from patients through our Klinik digital triage system. The successful candidate will work as part of our multidisciplinary team to assess, diagnose, and treat patients presenting with a wide range of minor illnesses and urgent care needs. They will play a key role in improving access to clinical services, supporting chronic disease management, and contributing to the overall health and wellbeing of our local patient population. Key Responsibilities Patient Care Work in partnership with GPs to provide high-quality, responsive triage decisions in a timely fashion and as set out in the practices PMS contract. Collaborate effectively with GPs, nurses, and other healthcare and social care professionals to ensure holistic and coordinated patient care. Assess patients, obtain detailed medical histories, and conduct comprehensive consultations as required to reach clinical triage decisions and treatment. Analyse, diagnose, and explain medical problems clearly and compassionately to patients and their families. Where appropriate, discuss with the referring or duty clinician doctor the needs of patients presenting with complex health needs or conditions requiring specific treatment, hospital admission or referral. Recommend, arrange, and interpret appropriate diagnostic tests and investigations in line with clinical governance and agreed protocols. Formulate evidence-based treatment and management plans, consulting or referring to a supervising GP as appropriate. Make direct referrals to primary, secondary and social services Educate patients and their families about health conditions, treatment plans, and preventative healthcare to promote self-management and long-term wellbeing. Ensure all medical equipment is maintained, clean, and in good working order. Accurately document all aspects of patient care, ensuring legal, clinical, and administrative requirements are met. Manage a defined caseload of patients through triage and/or booked clinics, taking responsibility for episodes of care within own scope of competence. Where appropriately trained, work with a high degree of autonomy including clinical assessment, diagnosis, treatment, and prescribing. For those developing towards advanced practice or prescribing roles, work under appropriate clinical supervision with opportunities for progression. Documentation and Administration Maintain accurate, timely, and comprehensive records of all patient assessments, treatments, and care plans in accordance with professional and organisational standards. Complete all documentation required for clinical, administrative, and legal purposes, ensuring compliance with data protection and confidentiality regulations. Learning and Development Take responsibility for personal professional development, demonstrating commitment to ongoing training and continuing professional development (CPD). Stay up to date with current medical research, evidence-based practice, and clinical innovation through study, professional meetings, and relevant training. Reflect regularly on clinical practice to identify learning needs and support continuous improvement. Confidentiality Handle all information relating to patients, carers, staff, and the organisation in a strictly confidential manner. Adhere to data protection legislation and practice confidentiality policies at all times. Share information only with authorised individuals and in accordance with policy and professional standards. Health & Safety Comply with all practice health, safety, and infection control policies and procedures. Maintain up-to-date knowledge of health and safety legislation and best practice. Use personal security systems in line with organisational guidance. Identify and manage risks within daily work activities, reporting hazards promptly. Participate in regular infection control and health & safety training (minimum annually). Actively promote safeguarding and the welfare of children and vulnerable adults.
