Practice Nurse

Job summary The Practice Nurse will provide high-quality nursing care within a primary care setting, supporting patients across a range of health needs. The role involves delivering nurse-led clinics, managing long-term conditions, and providing preventative care and health promotion. You will work both independently and as part of a multidisciplinary team to ensure safe, effective, and patient-centred care. Key responsibilities Clinical duties Management of chronic diseases including asthma, diabetes, and hypertension Cervical screening Immunisations and vaccinations (adult and childhood where applicable) Health promotion and preventative care Nurse-led clinics Patient care Provide advice, support, and education to patients and carers Promote self-management of long-term conditions Deliver care in a safe, respectful, and patient-centred manner Team working Work collaboratively with GPs, nurses, HCAs, and administrative staff Contribute to effective running of clinical services Participate in team meetings and clinical discussions as required Professional responsibilities Maintain accurate clinical records Work within professional guidelines and practice policies Maintain and develop clinical skills through ongoing CPD
