About this role

The postholder will support patients through their hospital care journey, providing the patient with a single named point of contact who will coordinate their care.

Work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients’ journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments

across the Trust.

We are seeking a proactive and resilient General Surgery and Colorectal Pathway Navigator to support the effective delivery of the service. This role is central to ensuring patients move smoothly and safely through their diagnostic and treatment pathways, working closely with clinical teams across multiple sites.

This is a busy, fast paced position that requires excellent attention to detail, strong organisational skills, and a commitment to delivering a high‑quality, patient‑centred service.

The successful post holder will need to demonstrate the ability to work independently, manage competing priorities, and communicate confidently with both patients and clinical colleagues.
