About this role
We are offering anexciting opportunityfor an experienced, innovative Paediatric Nurse to lead thePalliative and End of Life (EoL) Diana Nursing Pathwaywithin Newham Children’s Community Nursing Service. This places you at the heart of compassionate, community‑based care, supporting children with life‑limiting conditions and their families at the most critical moments of their journey.
The Palliative and EoL pathway lead works along the Core Nursing Pathway Lead and Complex Care Pathway Lead and sits within a wider multi-disciplinary service including Nursing (Specialist School Nursing, Asthma, Epilepsy, Continuing care) and Community paediatricdietitians. The Pathway Lead will also be expected to have an MDT approach to patient care with the psychology and play therapy services .
You will be responsible for the over site of palliative and EoL patients in the community setting, this may include holding a case load of your own as well as supporting and supervising the band 6 children's community nursing to deliver palliative nursing care to children and their families.

To provide expert nursing advice and support to children who have life limiting conditions or are requiring end of life care and their families across the borough of Newham.
To work as the nurse lead for the palliative and end of life nursing pathway for children as part of the Children’s Community Nursing service (CCNS) and act as an expert resource to staff working within all teams.
To lead on End of Life care planning and deliver care for children who are End of Life.
Work in partnership with the Symptom Control team at GOSH (or other as appropriate) to provide advice and support to children & young people with life limiting/life threatening illness and their parents, carers and other professionals working with them in the community setting.
Take responsibility for line management of junior members of the team who provide nursing care to children on the palliative and end of life pathway.
To work with the Complex Health Needs Nursing Pathway lead and the Core Nursing Pathway lead within the CCNS to ensure appropriate nursing care and interventions are provide to children and their families.
