About this role
The following are the core responsibilities of the Care Coordinator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. Assist clinicians with relaying and managing actions following blood results, this may be via phone and SMS. This will likely relate to providing a one-stop recall process, so patients reviews can be rationalised to avoid uneccessary appointments and duplication. b. Coordinate recall process for long-term conditions and reviews, ensuring all aspects of QOF and Enhanced Services are taken into account and that duplication is minimised. c. Assist with prescription queries and liaising with the clinical pharmacist team. This will involve reviewing queries and ensuring accurate tests (such as annual blood forms) are generated to avoid unnecessary delays or additional appointments. d. Distributing labs and prescriptions to GPs as required. Clear protocols and guidance will be available and appropriate training will be provided. e. a. Partake in audit as directed by the audit lead b. Support admin staff with other tasks cover during staff absences or high workload periods c. Book appointments d.
