About this role
We are seeking a proactive and compassionate Care Coordinator to join our team. This role plays a key part in supporting patients with long-term health conditions by coordinating their regular reviews and ensuring all aspects of their care are delivered by the appropriate clinicians. You will also take responsibility for managing accurate and timely patient registrations. As a Care Coordinator, you will: Manage patient recallsfor long-term condition reviews, ensuring patients attend regular check-ups and receive complete care from the relevant healthcare professionals. Support patient registrations, ensuring records are accurate and up to date as part of the onboarding process. Work with patients, families, and carersto improve understanding of long-term conditions. Provide a point of contactfor patients to help with queries, appointments, and access to high-quality information about their care options. Coordinate and navigate careacross services, ensuring patients receive joined-up support from the right professional at the right time. Collaborate with GPs and other professionalswithin the Primary Care Network (PCN) to proactively manage a caseload, including patients with long-term conditions, and refer them appropriately within the team.
