As a Rapid Response Support Worker, you'll step in at critical moments helping people with complex needs leave hospital safely and rebuild life at home with their families or step in to support stabilising a care package to enable clients to remain in their own home. You'll bring calm, confidence, and consistency when it matters most.

You'll support the set-up and stabilisation of both new and existing complex care packages, including
- Supporting safe and timely hospital discharges
- Providing high-quality personal care and daily living support
- Creating routines and helping clients regain independence
- Offering reassurance and continuity to families
- Mentoring new Support Workers and modelling best practice
- Liaising clearly and professionally with Care Managers and clinical teams
- Organising essentials such as medication, supplies, and care equipment
Flexibility That Works Both Ways
- 162 working hours per month
- Breaks between assignments - length depends on length of previous assignment
- Overtime maybe available if required
- Work live-in placements (sometimes 2+ weeks for continuity)
- Work live-out shifts (accommodation will be provided if not within daily traveling distance)
- Travel across the UK to support care packages

What We're Looking For
- Full UK Driving Licence
- Proven experience in complex care
- Confidence working independently
- Strong communication and organisation skills
- A calm, professional approach with clear boundaries
- Hospital discharge experience is desirable
- Level 2 Health & Social Care qualification or above is desirable
