Band 4 Diabetes Health and Wellness Coach | West London NHS Trust

About this role

The Diabetes Health and Wellness Coach will provide health improvement expertise and support people with healthier lifestyles through supportive self-management interventions, including patients with Type 2 Diabetes. The successful candidate will be helping people gain and use the knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals.

We are looking for an excellent team player who embodies a patient-centred approach to health coaching interventions and has a genuine passion for improving health outcomes for communities in Ealing. The role will also support the wider Healthier Lifestyles Team in promoting healthier lifestyles, behaviour change and health equity across the borough through community outreach and engagement.

· Supporting patients through one-to-one health coaching interventions to support self-management of long term conditions, including type 2 diabetes, with the use of coaching models and motivational interviewing.

· Attend monthly cooking classes with patients and other health improvement activities.

· Working alongside primary care, specialist care, social care and physical and mental health services.

· Use motivational interviewing and other behaviour change strategies to increase patients’ skills and confidence to manage their own condition and set achievable goals, engaging patients in making informed decisions.

· Provide weight management, physical activity, smoking cessation and alcohol awareness support.

· Establish and maintain strong and effective partnerships with partner organisations who can influence and deliver Health Improvement and Health Inequalities work e.g. health care staff, Local Authority and Voluntary Sector.

Provide health education to improve health, encourage healthy lifestyles and promote behavioural change.
