About this role

  • The Staff Nurse will deliver high quality, personalised, compassionate care to service users, providing a range of specialist interventions.

  • Band 5 Nursing roles are only open to staff with a Nursing registration, obtained as a result of completing a qualification recognised by the Nursing and Midwifery Council (NMC).

To support children and young people with complex medical needs at home, and in school. To support the paediatric continence service to hold clinics within two boroughs to manage bowel and bladder challenges.

Key Task and Responsibilities

  • Exercise a high degree of personal and professional autonomy and have the ability to plan and reach complex and critical judgments, using decision-making skills.
  • Deliver high-quality clinical care within the service. This will include acute and primary care, long-term conditions management, enhanced assessment and diagnostic intervention services, health promotion activities and be in line with the NHS Plan, public health indicators and National Service Frameworks.
  • Demonstrate and provide robust clinical skills with a sound understanding of evidence-based nursing practice to provide a pro-active approach to ensure quality and outcome-driven practice on a day-to-day basis.
  • Support the development of evidence-based practice in the specialist field and promote research as appropriate.
  • Participate in managerial and professional clinical supervision, training, and annual appraisal and ensure that this becomes an integral part of team practice.
  • Ensure the implementation of effective and appropriate patient-centred care-planning tools, long-term condition registers and consistent delivery throughout the service.
  • Work collaboratively with the Clinical Lead, GPs, and other clinicians to meet and review service needs as required.
  • Be guided by standard operating procedures (SOPs), good practice, established precedents and understand what results or standards are to be achieved. Make comments on proposals for changes, regarding working practices, service improvements or patient feedback.
  • Work independently within organisational and professional guidelines and established professional parameters, working to the nationally recognised code of conduct and Standards of Proficiency for Nurses. Work as part of the multidisciplinary team.
  • Communicate effectively, with sensitivity and compassion, and manage relationships with people, making reasonable adjustments where necessary.
  • Ensure the privacy, dignity and safety of individuals is maintained at all times.
  • Monitor the condition and health needs of people within their care on a continual basis in partnership with the members of the MDT team, families and carers.

Planning & Organisational Skills

  • Plan and organise programmes of care. Plan and organise patient care activities, reprioritising as required to ensure the interests of the patients are met.
  • Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating, or handing over responsibility for care.
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
  • Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of people first.
  • Assist in the implementation of appropriate action to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers.
  • Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.
  • Support people to improve and maintain their mental, physical, behavioural health and wellbeing.
  • Participate in discharge planning.
  • May organise staff and allocate duties during a shift to support staff and/or less experienced colleagues.

Patient/Client Care

  • Assess, plan, implement and evaluate clinical care/care packages within scope of practice.
  • Provide specialist advice concerning the care of treatment of identified groups of patients / service users.
  • Support the delivery of an effective integrated healthcare service clinical model.
  • Manage a complex clinical caseload.
  • Ensure all clinics and consultations are delivered in a clinically effective and timely way, in line with service needs.
  • Be able to give intravenous injections, immunisations and syringe pumps and infusions as necessary for the role.
  • Maintain systems for the collection of triage and allocation of referrals, as appropriate, monitoring and supporting the auditing of quality and effectiveness of service delivery.
  • Maintain an effective case management service to ensure care and continuity of treatment is maintained throughout the patient care pathway.
  • Attend and actively participate in team / care plan reviews.
  • Maintain close working partnerships with all services that may be involved with the care and management of the service users.
  • Understand and work within the boundaries of the Lone Working Policy and have an awareness of Personal Safety in all healthcare settings, including clear and updated communication with the shift lead.
  • Maintain timely and accurate patient written/electronic records in line with the Trust Policy, procedure and protocols. Use and complete care plans, pathway documents and RIO notes appropriately and in a timely manner.

Research & Development

  • Undertakes surveys or audits, as necessary to own work; may occasionally participate in R&D, clinical trials or equipment testing. Participate in clinical trials or complete staff surveys. Participate in audit and patient satisfaction surveys.
  • Contribute to clinical or governance reviews of the team’s activity and the monitoringofperformance. Contributetorelevantauditpracticedevelopment on research activitythat maybe takingplace in the work area.

Communication & Relationship Skills

  • Provide and receive complex, sensitive or contentious information, which requires tact and persuasive skills. Be able to motivate, negotiate, provide empathy or reassurance, where there are barriers to understanding or co-operation is required.
  • Provide and receive complex or sensitive condition-related information concerning the assessment, treatment and risk factors in relation to a range of health conditions.
  • Participate in clinical discussions/meetings regarding patients’ care and work collaboratively with a range of professionals in statutory and non-statutory organisations and agencies.
  • Liaise and communicate with staff, managers, patients, other professionals and service users at all levels.
  • Ensure that confidentiality is maintained in conjunction with the Trust's Confidentiality Policy.
  • Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family.
  • Liaise with other senior Trust personnel to share expertise, promote ideas and ensure the service is working collaboratively with other teams, as required.
  • Develop and maintain inter-disciplinary and inter-agency working, with all relevant organisations and agencies.
  • Engage in reflective practice, to ensure effective patient care is based on evidence-based practice and with trust values in mind. Attend staff training and meetings including reflective practice groups and activities, including the development of a reflective portfolio.

Analytical & Judgement Skills

  • Be able to consider a range of facts or situations, requiring analysis or comparison of a range of actions.
  • Decide on the implementation of care programmes where there are a number of options, using clinical judgement on problems requiring investigation and analysis, or responding to deteriorating patient condition or clinical emergencies such as anaphylaxis, utilising skills learnt to assist in emergency situations.
  • Carry out clinical assessments within the sphere of expertise and identify and escalate any issues relating to poor clinical standards or clinical risk.

Human Resources

  • Provide advice or demonstrate own activities or workplace routines to new or less experienced employees in own work area.
  • Provide day to day supervision, mentoring, training and support to less experienced staff members or students.
  • Allocate work to clinical support workers or students.
  • Regularly responsible for providing training to Nursing and other disciplines where the topics or subjects are connected with own work area.

Financial & Physical Resources

  • May handle or process cash, cheques, patients’ valuables.
  • Responsible for the safe use of equipment other than equipment which they personally use. Responsible for equipment for use by others, use of equipment by others e.g. use of hoists, surgical equipment
  • Maintain stock control, order supplies. Responsible for the safe custody and storage of drugs, sharps and clinical equipment, reporting all incidents in line with Trust policy.
