Specialist Palliative Care Nurse

About this role

Main duties of the job The post holder will be able to : • To act as an expert nurse/Practitioner for patients and families and be a resource for advice and consultancy to empower other professionals to provide high quality palliative/end of life care. • To work autonomously to provide holistic assessment and management for patients with complex problems, and their families, enhancing their quality of life and enabling them to make choices about their care. Monitoring, evaluating and reviewing palliative interventions. • To act as a patients advocate and support informed discussions around choices concerning treatment options affecting quality of life and preferences around place of care and place of death. Interviewer information pack • To work collaboratively as part of a multi-disciplinary team supporting colleagues and respecting their contribution to meetings. • To have organisational knowledge relating to Trust protocols and procedures and adhere to them. • To be responsible for providing accurate records of information required by the Trust. • To contribute to quality initiatives and service development. • To have a sound clinical knowledge on all aspects of palliative care and palliative medicine with awareness of the benefits and reactions to treatments and drugs used for symptomatic and therapeutic benefit, providing expertise in symptom control and offer appropriate advice as required Working for your organisation Our Values 2025 - 2030 Working with our colleagues, patients and carers we have co-produced three new values that we aim to embed within our culture here at NELFT.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex. Probationary Period This post will be subject to a probationary period. High Cost Area Supplement - Outer London This post also attracts payment for High Cost Area Supplement of 15%of the basic salary (minimum payment of £4,870 and a maximum payment of £6,137 - per annum, pro rata for part time
