A Vacancy at St George's University Hospitals NHS Foundation Trust.
To provide expert care and clinical leadership in managing Parkinson's Disease and associated Movement Disorders. The role involves working autonomously within a multidisciplinary team to provide expert patient centred care and drive service innervation.
The role includes the role of the Deep Brain Nurse Specialist and is involved in delivering non oral therapies for those with Parkinson's Disease.
Offering leadership and service development within Movement Disorders service, additionally managing a small number of Neurology based clinical nurse specialists.
To act as a patient advocate, ensuring a person-centred approach.
Oversee the patient’s treatment and investigation pathway, ensuring it is seamless and well communicated to all parties, including the patient.
Provide practical and emotional support to the patient and family.
Coordinate the provision of information and ensure that it is timely, and meets the current needs of the individual patient and family/carers. Ensure that the information has been understood and that where a patient information prescription is available that this is used.
Ensure a holistic needs assessment of the patient is undertaken and that a written care plan is provided / updated.
Liaise with health and social care agencies and professionals in the community, including the primary care team.
Support the acquisition of new skills by the patient as needed, for example, care and management of Deep Brain Stimulators, infusion devices and pumps.
Liaise with the nursing and allied health professionals in clinical departments as they move between care settings along the patient pathway.
To lead on Patient Information Provision for the MDT. Ensuring in-house patient information is up to date.
To be the delegated lead for patient involvement in the MDT and use patient feedback to help improve services.
Understand the training needs within the service and support staff in their own development plans
