Six Month Stroke Review Coordinator

To undertake all aspects of the six month stroke review duties, including planning home visits, undertaking blood pressure, maintaining databases and completing reports. To provide stroke education and information, to relatives and carers, disciplines and agencies. To participate in multidisciplinary / multi-agency team meetings and case conferences, to ensure the co-ordination of patient care. This may include the review of patient progress and discharge planning. To maintain accurate clinical records and update databases relating to the six month stroke reviews. To complete the standardised stroke review questionnaire and to summarise the information in written format to the patient and their GP. To produce patient letters and reports for GPs and Stroke Consultants. To input to databases as required, including the SSNAP database. To carry out therapy assistant duties as required
