Be professionally and legally accountable for all aspects of own work, including the management of your patient clinical episode of care. Undertake a comprehensive assessment of patients physical needs, while appreciating the psychological and emotional impact in relation to their diagnosis and symptom profile, understand the diverse and complex presentations and multiple pathologies; use advanced clinical reasoning skills to provide an accurate understanding of their condition. Formulate accurate prognosis and recommend best course of intervention, developing a self-management discharge plans where appropriate. Assess the patients understanding of treatment proposals, gain valid informed consent if appropriate and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment. Evaluate patients adherence to management plans, monitor progress, reassess and modify management plans as appropriate. Provide or direct patients to accredited information and supportive literature to assist in their management Ensure the Safeguarding of vulnerable adults and children in line with safeguarding policy and procedure.
