Specialist Nurse for Integrated Adolescent Service (IAS)

This is a unique opportunity for a skilled SCPHN nurse to lead and deliver high-impact, community-based care for vulnerable adolescents within the Integrated Adolescent Service (IAS). The post holder will undertake holistic health assessments, identify unmet needs, and deliver targeted, trauma-informed interventions that improve health outcomes and reduce inequalities. Working at a senior level within a multidisciplinary team, the role will provide expert health input into safeguarding and youth justice pathways, influence care planning, and strengthen multi-agency collaboration. The post holder will act as a key advocate for young people, improving access to services and driving engagement with those who are often hardest to reach. The role also contributes strategically to service development, evaluation of the IAS pilot, and shaping future models of care, while maintaining high standards of clinical governance, safeguarding, and professional practice.
