Responsibilities
Conduct comprehensive assessments of individuals and families to identify their needs and risks.
Develop, implement, and review personalized care plans in collaboration with clients and other professionals.
Provide expert advice and support to clients to enable informed decision-making and improved outcomes.
Work closely with multidisciplinary teams, including health, education, and housing services, to coordinate care and support.
Maintain detailed case records and prepare reports in compliance with legal and organizational standards.
Advocate for clients’ rights and access to services while ensuring safeguarding policies are followed.
Participate in supervision, training, and professional development activities to enhance skills and knowledge.
Requirements
Degree in Social Work or equivalent qualification recognized by the relevant professional body.
Current registration as a Social Worker with the appropriate regulatory authority.
Minimum of two years post-qualifying experience in social work practice, preferably within a community or healthcare setting.
Strong assessment and analytical skills with the ability to manage complex casework effectively.
Excellent interpersonal and communication skills, capable of building rapport with diverse clients and stakeholders.
Demonstrated understanding of safeguarding principles and relevant legislation.
Ability to work independently and as part of a multidisciplinary team under pressure while maintaining professionalism.
