Job Description Practice Nurse We are looking for a qualified and experienced Practice Nurse to join our friendly and supportive team. The successful candidate will play a key role in delivering nursing services, supporting chronic disease management, and promoting health and wellbeing in our local community.
Key Responsibilities
Deliver high-quality, evidence-based nursing care to patients of all ages. Provide services including immunisations, cervical screening, wound care (general nursing). Support the management of long-term conditions such as asthma, COPD, diabetes, and hypertension through regular clinics and monitoring. Conduct health checks and screening, including NHS Health Checks and lifestyle assessments. Collaborate with GPs and the wider clinical team to plan, implement, and evaluate care. Maintain accurate, timely documentation in line with clinical governance and confidentiality standards. Ensure compliance with infection control, safeguarding, and health & safety policies. Participate in quality improvement initiatives, audits, and clinical meetings.
Essential Requirements
Registered Nurse (RGN) with valid NMC registration. Experience working in a primary care or community setting. Excellent clinical, communication, and organisational skills. Ability to work independently and as part of a team. Commitment to continuing professional development.
Desirable
Additional qualifications in chronic disease management, cytology, and immunisations. Experience using clinical IT systems (e.g., EMIS Web, SystmOne).
